The Law Offices of Bradley J. Frigon can offer assistance as needed to seniors or individual's with special needs in a professional manner while maintaining dignity for the elderly and their families. We are an advocate for our special needs and senior clients and strive to maintain a continuum of care to prevent unnecessary hospitalizations and costly crises.
When the activities of daily living become an effort or unsafe for our frail elders, the chronically ill, and the physically disabled, the challenges of our care manager begin. It is the goal of the care manager to find the support our clients need to live in the least restrictive community setting of their choice through the enablement of community, family and personal resources. Our Care Manager works hand in hand with professional practitioners such as lawyers, physicians, accountants, financial planners, health care and long term care agencies as a team to create a plan for the individual.
On a daily level, our care manager may offer placement services, meet with health care providers, develop and implement a care plan, coordinate health care service providers and monitor progress of the client. In many cases, our care manager becomes the client's advocate, or the contact to the outside world for the institutionalized client.
As an Elder Law firm, the Law Offices of Bradley J. Frigon provides a continuity of services within a team framework. Our attorneys create and implement the asset protection, estate plan or guardianship and conservatorship proceedings and our care manager will develop and implement a care plan, monitor and coordinate care services, interface with health care providers, assist with placement decisions and maintain contact with other family members.
